Enhanced Services Program

Are you or your family enrolled in Medicaid Managed Care? If so, we can offer you enhanced support for things like housing services, nutrition, or transportation through the NYS Medicaid 1115 waiver program.

 

 

The Enhanced Service Program (ESP) is designed to help NYS Medicaid managed care members access resources to address unmet health related social needs.

Services

Housing

For

Medicaid Managed Care members who need assistance to overcome homelessness and housing disparities.

Provides

  • Pre-tenancy

  • Tenancy-sustaining

  • Rent/temporary assistance

  • Utilities

  • Community transitional support

  • Home safety remodification

Nutrition Pantry Stocking

For

Eligible women enrolled in a managed care plan, to supplement their daily nutritional needs.

Provides

  • Fresh produce

  • Non-perishable groceries

Transportation

For

Our members to help them complete goals set by their Social Care Navigators/Case Managers.

Provides transportation assistance to

  • ​Educational or justice institutions

  • Non-medical appointments

  • Housing searches and viewing
