More than 20 Community partner organizations are embedded at Harvest House, working side-by-side to provide an ecosystem of support that helps build a stronger east side community. Our community navigation specialists help our community gain access to services at both Harvest House and throughout the community.

Our Trauma Informed Leadership Model brings community together to build and facilitate person-centered solutions that have a positive impact on health, well-being, and quality of life, and ultimately break cycles of poverty, hopelessness, and inequity. 

 

Our model is built on providing access to all 12 Social Determinants of Health (SDOH) - the conditions where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.  

Access to Housing

Improves access to affordable, safe housing.

On Buffalo’s East Side, housing costs are a burden for most low-income renters – 82% spend a third or more of their income on rent.  A lack of access to permanent housing exacerbates disparities in health outcomes. Our homeless prevention programs assist families with access to safe, affordable housing, case management, and support services, preventing homelessness and building economic stability.

Access to Medical & Dental Care

Provides Access to Quality Healthcare for Individuals who are Uninsured, Underinsured, and the Working Poor.

Subtotal Medical & Dental served 2024: $1,040 

Annual Economic Impact: $2.6M 

The working poor have reduced access to resources such as healthcare, opportunities to exercise, and healthy foods, which contribute to a higher risk of developing poor health. Our Good Neighbors Healthcare brings licensed medical, dental, and healthcare insurance providers together with Medical, Dental, Social Work, Nutrition, and Psychiatry students from SUNY Buffalo to provide access to basic health services and help enroll our neighbors in health insurance programs. Without this support, our patients' only access for non-critical care will be the hospital waiting rooms. 

Food As Medicine

Provides fresh fruits and vegetables to low-income individuals who are food insecure

47% of senior citizens living around Harvest House are food insecure. Twice each week, our neighbors receive fresh fruits and veggies, chat with a nurse, have their blood pressure taken, and try a new recipe served by a nutritionist. Our navigators also help link our neighbors to other services. A healthy diet has been shown to reduce cancer, diabetes, heart disease, stroke, and Alzheimer's. It’s wellness in action!

Access to Basic Needs for Women & Children 

Strengthens families living in poverty

4,895 children 

Annual Economic Impact: $2.7M 

More than 17,000 children living on Buffalo’s East Side (nearly 50%) live in poverty. The effects of childhood poverty can be devastating and include poor health, education, and employment outcomes.         

 

In addition to providing families with food, diapers, clothing, educational toys/books, and supplies, health services agencies rely on Harvest House to navigate pregnant women, coming to us for their children’s needs, with prenatal and perinatal healthcare services.

 

Together, we are helping reduce high infant mortality rates and improve maternal and infant health in our community. 

Access to Living Wage Jobs 

Help adult learners get comprehensive training to secure living-wage jobs.

Harvest House partners with workforce development agencies to embed life and work skills training into core training programs. Students at our New Hope Education Center have access to all SDOH initiatives on-site. We developed a trauma informed care curriculum that helps deepen students understanding of ways in which health disparities and trauma impact their ability to learn, grow, build trust, and lead healthier lives. Graduates earn an average of 33% more income!
